The Care Assessment
What does an elder care assessment accomplish?
When performing a comprehensive elder care assessment, we examine various factors of the seniors home environment and physical and mental status. We carefully assess the senior's:
- Physical functional abilities,
- Medical condition,
- Mental status,
- Physical surroundings,
- Financial resources,
- Social and peer interactions, and
- Family involvement.
We evaluate the elderly adult in his or her own home environment, which gives us an accurate understanding of the client's situation.
A comprehensive assessment includes:
- Initial phone interview to communicate basic information about the elderly person's needs and how our services work;
- In-home evaluation of the client's home pertaining to mobility and safety concerns;
- Discussion about current health concerns, social and family support system, and transportation needs;
- Review of medical history, current medical concerns, medications, lab work, and diagnostic test results;
- Evaluation of the client's ability to make decisions, manage personal finances, and exhibit appropriate behavior;
- Assessment of cognitive brain function for Alzheimer's and other dementia disorders;
- Review client's current Advanced Directives or help develop one;
- Observation of current maintenance needs of the home environment;
- Referral to an elder law attorney should there be a need; and
- Determination of personal care needs such as home health care services vs. placement in an assisted living facility (ALF) or a nursing home.

Elder Care Plan
Once the elder care assessment has been completed, a comprehensive written Elder Care Plan is developed.
Recommendations and referrals are made to meet the unique needs of each individual client. There is no one size fits all.

A care plan, which includes recommendations and referrals, is presented to the elderly client and/or family members. The care manager implements and oversees all aspects of the care plan. Reports will be provided to the family as arranged between the care manager and the family.
